STUDENT AND PUBLIC ACCIDENT/INCIDENT REPORT

See Something, Say Something If you witness an injury, unsafe condition, suspicious activity, or any incident involving a student, visitor, or member of the public on campus or during a school-sponsored activity, you are responsible for reporting it. Prompt action protects everyone and ensures proper documentation.
Follow the steps below to ensure a timely response and accurate reporting.

STEP 1: CALL FOR HELP (EMERGENCIES ONLY)

If the incident is an emergency, the first person to become aware of it or arrive on the scene, whether an employee, staff member, or visitor, must immediately contact SCCCD Police Dispatch:

  • SCCCD Police Dispatch: (559) 244-5911

SCCCD Police will assess the situation and contact Emergency Medical Services (EMS) if needed.

Note: Campus personnel are not permitted to transport injured individuals.

STEP 2: DETERMINE REPORTING PATH

Off-Campus Injuries (Employees, Student Workers, Clinical Rotations) If the injured person is an employee, student worker, or a student injured off campus during a clinical rotation (e.g., nursing student at a hospital), do not use this form.

Instead, call the Company Nurse Hotline: 877-854-6877

On-Campus Injuries (Students, Visitors, Public) All currently enrolled and registered Reedley College students who are injured or become ill on campus or during a school-sponsored activity may be eligible to file a medical claim.

This includes emergency transport through the Student & Athlete Insurance Network (SAIN) and/or other campus coverage.

Contact Health Services to speak with a College Nurse for more information.

STEP 3: NON-EMERGENCY INQUIRIES AND SAFETY REPORTING

For non-emergency questions, health concerns, or to report safety issues or suspicious activity, contact:

SCCCD Police Department

  • Phone: (559) 244-6140
  • Location: 1940 N. Calaveras Street, Fresno, CA 93704
  • Website: SCCCD Police Contact Info

Reedley College Health Services

  • Phone: (559) 494-3028
  • Location: Student Services Building #30

Reminder: If you see something, say something. Report any concerning behavior, safety hazard, or incident to SCCCD Police immediately.

STEP 4: COMPLETE THE INCIDENT REPORT FORM

SCCCD Public Accident Form

Who completes the form: The main employee witness, any other witnesses, or the first employee who becomes aware of the incident is responsible for completing the form. All individuals with direct knowledge of the event should contribute to ensure accurate documentation.

Where to send the form: Submit the completed form to Health Services within 24 hours of the injury or incident.

FINAL REMINDER

See Something, Say Something. Whether it’s an injury, suspicious behavior, or a safety concern, report it immediately to SCCCD Police. Your awareness and action help keep our campus safe.
