LVN to RN Multi-Screen Criteria

All individuals who meet minimum enrollment requirements are eligible to apply to the nursing program. Criteria and complete information on the selection process as well as the mission and philosophy of the program are described in this Application available in Admissions and Records at the Madera Community College Center only and online at the Madera Community College Center website (click on Courses & Programs, then click on Health Science).

This program is designed for completion by Licensed Vocational Nurses, and it requires one academic year beyond completion of the prerequisite courses, non-Nursing breadth requirements. LVN to RN students enters the program after completion of RN 160 and all other program prerequisites. Graduates earn an Associate in Science Degree upon completion of designated courses. Communication, natural and social science, and nursing courses must be completed with a grade of “C” or better to be eligible for licensure requirements of the Board of Registered Nursing. Science prerequisites (Bio 20, Bio 22, and Bio 31) must be completed within the last five (5) years.


Applications must be submitted during the specified application period. Overall GPA must be 2.5 in specified prerequisite courses and Test of Essential Academic Skills (TEAS) with a cut score of 62% or higher to qualify for the RN candidate pool.

Applications will be accepted December 1, 2017, thru February 2, 2018, at 3 pm. All applicants must provide a current, working email address with their application. It is the responsibility of the applicant to maintain a working email address and to notify the nursing counselor of any changes. Students who have been accepted into the program will be notified by email. Students will be required to confirm acceptance of admission via email within two weeks of the selection date.

PREASSESSMENT TESTING REQUIREMENT

The Test of Essential Academic Skills (TEAS®) is a multiple-choice assessment of basic academic knowledge in reading, mathematics, science and English and language usage. The objectives assessed on the TEAS exam are those which nurse educators deemed most appropriate and relevant to measure entry level skills and abilities of nursing program applicants. To be eligible for the pool, applicants must have taken the ATI TEAS and achieved a score of 62% or higher and submit official transcripts of those scores. TEAS scores before 8/1/16 will not be accepted. Candidates must purchase and schedule TEAS testing. Go to www.atitesting.com for instructions on purchasing official TEAS transcripts.

ADMISSION TO THE LVN TO RN ASSOCIATE IN SCIENCE DEGREE NURSING PROGRAM

Applicants are responsible for preparing a complete packet which includes the following:

  • Sealed official transcripts verifying completion of all prerequisite coursework. Official transcript from the LVN education/training program that the candidate completed. To determine equivalency of coursework from another college, course descriptions and a syllabus or topical outline of the course must be included in the packet. Students who have completed courses outside of the United States will need to complete a formal foreign record evaluation if any of the courses will be used to fulfill graduation requirements. An official sealed report from an official accredited educational document service must be included in the application packet.
  • Verification of high school graduation or equivalent: official high school transcript, G.E.D., or verification of an academic degree from an accredited college/university.
  • Verification of active LVN licensure and graduation from an accredited LVN program.
  • Verification of completion of the Test of Essential Academic Skills ATI TEAS with a score of 62% or higher. TEAS scores before 8/1/16 will not be accepted.
  • Completed Madera Community College Center LVN to RN Associate in Science Degree Nursing program application.

It is the responsibility of the applicant to provide official documentation of prior coursework, experience, licenses, and certificates.

Selection Procedure:

The application packet and all requested supplemental documents must be received on or before the stated deadline. All prerequisite requirements must be completed before applying to the program with a cumulative GPA of 2.5 or higher. Selection of applicants for admission to the LVN to RN Associate in Science Degree Nursing program is done through an evaluation process based on multi-screening criteria.

  • 85 - 100% = 35 points
  • 75 - 84% = 30 points
  • 70 - 74% = 25 points
  • 62 - 69% = 20 points
  • Two or more attempts (Minus 5 points per attempt).

Criteria 4. Prior Healthcare Work/Volunteer Experience

  • LVN licensed with one-year acute care work experience (inpatient hospital that includes direct patient care experiences in an LVN role) = 10 Points
  • LVN licensed with long term care work experience in an LVN role (skilled nursing facility with direct care experience or 100 hours of volunteer hours in healthcare) = 8 points
  • LVN licensed with clinical work experience in an LVN role (home health/hospice, school nurse aide, correctional facility, state, hospital, outpatient clinical setting, research, non-direct patient care LVN roles) = 6 points
  • LVN licensed with no work experience as an LVN = 4 points

  • Bachelor’s degree or higher = 5 points
  • Associate in Arts or Science degree = 3 points

Criteria 6. Completion of General Education toward Associate in Science Degree

  • All General Education requirements completed before submission of application = 10 points
  • All General Education requirements completed before start of RN program = 5 points
  • One or more General Education courses in progress = Zero points

Multiple experiences/circumstances will get the maximum points available.

  • Disabilities = 5 Points
  • Disadvantaged social or educational environment (EOPS) = 5 Points

  • Low family income = 5 Points
  • Veterans Status (Veteran or eligible spouse) = 10 Points

**Falsification of any documentation will disqualify applicants from entering the program.

REVIEW OF REQUIRED DOCUMENTATION FOR MULTI-SCREENING CRITERIA

Criteria 1:

Official, sealed college level transcripts from an LVN School of Nursing and all lower and upper division courses completed at regionally accredited academic institutions in the United States will be required before making an appointment with the Nursing Counselor. Official transcripts will not be required for courses completed at Reedley College, Clovis Community College, and Fresno City College. Failure to submit transcripts with posted grades will result in ineligibility for admission.

Criteria 2:

The same documentation for Criteria 1 is required for Criteria 2. **College level math accepted if intermediate algebra is not required based on placement tests. See a counselor for more details.

Criteria 3:

Students who have taken the ATI TEAS and achieved a score of 62% or higher) may submit official transcripts of those scores. TEAS scores before 8/1/16 will not be accepted. Five points will be deducted with each attempt to achieve 62%. Candidates must purchase and schedule TEAS testing. Go to www.atitesting.comfor instructions on purchasing official TEAS transcripts.

Criteria 4:

Work/Volunteer experience letter must be completed from current or former employer/volunteer organization verifying work/volunteer experience and must be within 12 months of the date of application. The letter must have an original signature and include the applicant’s name, start and end date, list full or part time employment, the number of hours worked or hours volunteered, job title, examples of duties to include patient interactions.

Criteria 5:

Official, sealed transcript from regionally accredited academic institution in the United States with degree posted.

Criteria 6:

All General Education requirements completed prior to submission of application or before start of RN program. One or more General Education courses in progress are zero points. All Science courses must be completed before submitting the application. LVN 30 unit option applicants are selected on a space available basis. LVN to RN Associate in Science degree applicants will be given priority for admission.

Criteria 7: Life Experiences/Special Circumstances

  • A disability is the same meaning used in Section 2626 of the Unemployment Insurance Code. Documented disability from college learning disability or disability support programs and services (DSP&S).
  • Disadvantaged social or educational environment (EOPS, TRIO, CalWORKs, Promising Scholars, Dream Act participants). A brief, personal written statement explaining situation or circumstances.
  • First generation to attend college: Letter from school authority (EOPS, Upward Bound/TRIO, High School Counselor, High School Teacher) or clergy verifying first generation status.
  • Language proficiency other than English: Letter from a professor, clergy, or supervisor stating applicant can speak, read and write in the native language. Transcripts showing completion of three years of high school foreign language with a “C” or better or two semesters of college-level foreign language. Languages accepted are: American Sign Language, Arabic, Chinese (including various dialects), Farsi, Russian, Spanish, Tagalog, and Languages of the Indian subcontinent and Southeast Asia.
  • Veterans/Dependent Status: Copy of DD 214 showing honorable discharge from military service.

Low family income: Proof of eligibility or proof of receipt of financial aid (Board of Governor fee waiver, Cal Grant, Pell Grant or other federal grant, CalWORKs)

**Falsification of any documentation will disqualify applicants from entering the program.

Students accepted to the nursing program are required to complete a criminal background check and drug screen. Any conviction on your clinical background check within the past seven years, including DUI, has the potential of denying you access to Fresno – Madera area healthcare facilities for clinical rotations, which may prevent you from being admitted to the nursing program. Any conviction on your background check may prevent you from taking the licensure examination and obtaining a license to practice nursing in the state of California. Please consult with the Director of the Nursing Program prior to application.
